First, the Thing Nobody Tells You About “Normal”

Before any individual marker, understand this, because it reframes everything else: a hormone blood test is a single snapshot of a moving target. In the perimenopausal years, estrogen and other hormones fluctuate dramatically — day to day, sometimes hour to hour. A level drawn this morning can look completely different from one drawn next week, and both can fall inside the “normal” reference range.

What Each Marker Actually Measures

FSH (follicle-stimulating hormone). Released by your pituitary to tell the ovaries to mature an egg. As the ovaries become less responsive in perimenopause, the pituitary shouts louder, so FSH tends to rise. A high FSH can support a perimenopause picture — but because it swings so much month to month, one reading is not definitive.

LH (luteinising hormone). Also from the pituitary; it triggers ovulation. Like FSH, it tends to rise and become erratic during the transition. It’s usually read alongside FSH rather than on its own.

Estradiol (E2). The main form of estrogen in your reproductive years. This is the one that fluctuates most wildly in perimenopause — it can be low, normal, or even unusually high depending on the day. A single estradiol number tells you very little in isolation, which surprises women who expected it to be the answer.

Progesterone. Produced after ovulation, it has a calming, sleep-supporting effect. It tends to decline earlier and more steadily than estrogen in perimenopause. Timing matters enormously: to mean anything, it should be measured in the second half of your cycle (around 7 days before your expected period), because it’s naturally low at other times.

Testosterone. Yes, women make it too, and it contributes to energy, mood, and libido. It declines gradually with age. Levels are often measured as “total” and “free” (the portion actually available to your body).

DHEA-S. A hormone from your adrenal glands that acts as a building block for other hormones. It’s sometimes included to get a fuller picture of the adrenal contribution, and it declines with age.

Why Timing Changes Everything

Progesterone measured in the first week of your cycle will look low because it’s supposed to be low then. Estradiol shifts across the cycle too. But “erratic” is the key word. A tidy, textbook progression is not what most perimenopausal panels show — and that messiness is normal, not a sign the test failed.
