The early stage: when progesterone leads the change

Progesterone typically declines before estrogen does. In the early stage of perimenopause — which for many women begins in the early 40s — estrogen levels can still be relatively normal while progesterone has already begun to drop.

The symptoms that cluster here are progesterone-driven: the background anxiety that feels slightly out of proportion to circumstances, the sleep that becomes lighter or more disrupted without obvious cause, the mood reactivity that feels a few degrees off from usual. Cycles are often still regular. Hot flushes may not have appeared yet. By conventional markers, nothing has changed.

The middle stage: when estrogen begins to fluctuate

As the transition progresses, estrogen levels become more variable — swinging higher and lower unpredictably before their eventual consistent decline. This fluctuation phase is often the most symptomatic period, because the body is adapting to a moving target rather than a stable one.

This is when hot flushes typically arrive, when cycle regularity begins to shift, when cognitive symptoms become more noticeable. The experience of good weeks and harder weeks — sharp days and foggy days, with no clear external reason for the difference — tracks the estrogen fluctuation directly. The late stage: the final transition

In the final stage of perimenopause, estrogen levels decline more consistently and cycles become increasingly irregular — longer gaps between periods, lighter or heavier flow, or both. This stage typically lasts one to three years before the final period.

The symptoms that are most pronounced during estrogen fluctuation may begin to stabilise somewhat as the fluctuation itself reduces and estrogen settles lower. Some women find this stage more manageable than the middle stage. Others find the sustained lower estrogen level introduces new challenges, particularly around bone health, cardiovascular markers, and vaginal changes.

Post-menopause: a new baseline

After twelve months without a period, you’ve reached menopause. Post-menopause is not the end of the hormonal story — it’s a new chapter with its own health considerations. Bone density protection becomes a priority without estrogen’s protective effect. Cardiovascular risk factors shift. The cognitive changes that were fluctuating during perimenopause often stabilise.

Many women describe post-menopause as a clarification. The unpredictability of the transition period is over. The body has found a new equilibrium.
